LivingWell Fees for Service

Not all services provided through LWIS are reimbursed by third party payers and may be considered to be wellness and/or not medically necessary. Verification of insurances and coverage is available, but does not guarantee payment. Payment plans are available if necessary.

A complimentary phone consult is available to determine if services are appropriate.

Basic IHP   - $150

LivingWell consult with a physician, recommendations to clinical and community resources and accessibility to LWIS supportive services

Complex IHP  - $500

LivingWell consult with 2 Physicians, a team meeting, and a written IHP including recommendations for clinical and community resources and accessiblity to a HOPE coach for ongoing support.

Comprehensive IHP  - $500 plus

$50 - $150 per hour dependent upon services.

Integrative Health Consulting  - $120 per hour
